Exciting News for Medicare Patients in Long-Term Care Facilities and for You!

SilverCare ACO is on a mission to revolutionize healthcare delivery for residents of long-term care facilities by closely collaborating with medical practitioners like you. Joining SilverCare ACO offers you the chance to be part of a movement aimed at achieving superior healthcare outcomes for long-term care residents, all while increasing revenue opportunities for healthcare providers.

When quality and participation goals are met, participating providers stand to gain from the savings recognized by CMS for the ACO. As a member of SilverCare ACO, providers can earn a percentage of the savings generated, tailored to the specific patient demographics they serve (subject to overall ACO savings and meeting quality and participation standards). Additionally, SilverCare ACO  partners with best-in-class solutions and provider partners. Starting from 2025, SilverCare ACO will also incorporate services such as palliative care, wound care, mental health support, and telehealth consultations.

Our organization is committed to providing unparalleled support to LTC physicians, empowering you to deliver top-notch care to the unique, often underserved, and high-needs populations you serve.
