Since its establishment in 2024, SilverCare ACO has been dedicated to addressing the specific needs of elderly individuals, as well as those managing Alzheimer’s disease or dementia, depression, amputations, developmental or physical disabilities, renal failure, and multiple chronic diseases and complications.

As a frontrunner in the long-term care sector, SilverCare ACO is committed to maximizing the benefits of an Accountable Care Organization, with a deep commitment to the success of its participating providers.
