Since its establishment in 2024, SilverCare ACO has been dedicated to addressing the specific needs of elderly individuals, as well as those managing Alzheimer’s disease or dementia, depression, amputations, developmental or physical disabilities, renal failure, and multiple chronic diseases and complications.

Through SilverCare ACO, physicians and long-term care facilities receive steadfast support from a seasoned team dedicated to enhancing healthcare standards for residents under their care.
We support long-term care physicians and facilities in adopting value-based care by conducting thorough analyses and offering actionable data and insights. These efforts drive enhancements in both quality and cost efficiency. Additionally, unlike other managed Medicare programs like Medicare Advantage Institutional Special Needs Plans (I-SNPs), providers within SilverCare ACO face no downside risk and are not required to make an initial capital contribution for participation.
